Swansea Bay University Health board has agreed boundaries to the practice area within which the Practice is contracted to provide General Medical Services to patients registered with the practice. Any person living within the practice area is eligible for registration.

The practice area extends from Briton Ferry roundabout in the West to the junction of the A48 with Water Street in the East.

The following areas are included in the Practice area:

  • Sandfields
  • Aberavon
  • Velindre
  • Margam
  • Goitre
  • Baglan

Please note: Cwmavon is no longer in our Practice area.  When we agree our new practice boundaries with the Health Board we gave an undertaking not to remove patients registered with the practice and resident outside of our revised Practice area.  However, registered patients moving outside the revised practice area will be required to register with another practice, there will be no exceptions to this rule.  We would like to apologise to any person affected by the change to our Practice area which was necessary to manage our workload.

 

As long as you are living within the practice boundaries you can register with the practice.

New patient packs can be collected within the opening times of the Practice. Completed packs need to be returned by the patient on a Thursday between 13:30 - 16:00 hrs to be able to join the Practice. You can also register by filling in the form at the foot of this page to submit an applicaiton online. 

Completed registration forms must be handed in at reception, any patient or their representative over 18 years of age must provide the Practice with some form of identification for the patient who wishes to be registered i.e. birth certificate, passport, driving license or utility/household bill stating an addreess registered withing our practice boundaries. 

All patients will need their NHS number to register at this Practice.

Prior to joining the Practice please ensure you have at least 1 month supply of medication from your previous practice.

Please be reassured, all information supplied is completely confidential. At the time of registration, you will be invited in to attend a new patient health examination. It is very important that you attend this appointment as we may not receive your full medical records for up to 8 weeks.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
