District Nursing Services

 

 

Our district nurses provide a healthcare service to people who are unable to visit the GP surgery or community clinics and who need nursing care, advice and support due to being permanently or temporarily housebound.

District nursing is a branch of the nursing profession consisting of specialist district nurses, community staff nurses and health care support workers with a wide range of specialist skills. The district nursing service works in partnership with colleagues in the community and hospitals to ensure the best care for patients. We also have specialist nurses and therapists as part of the team in order to enhance the care we can provide.

The aim of the 24-hour, seven-day-a-week service is to enable individuals to maintain their own independence and achieve self-care.

When this is not possible dignified and compassionate care is provided for those patients unable to return to independence, with chronic debilitating conditions and at the end of their lives.

Referrals are accepted from a variety of sources including:

  • Self referral
  • Hospitals -(local and regional)
  • General Practitioners
  • Other disciplines and agencies
  • Primary Health Care Team members
  • Carer’s/Family members

Referral Criteria

There is no waiting list for admission to the district nursing service and all patients are accepted providing that they are accessed as requiring nursing intervention. Patients may have acute or chronic medical or surgical conditions including:

  • Palliative Care
  • Terminal illness
  • Wound care
  • Ulcer and pressure area care
  • Administration of medication requiring injection
  • Health Promotion
  • Care advice and support

Neath Port Talbot Community Resource Team

The Community Resource Team (CRT) is a joint service provided by this health board and Neath Port Talbot council social services.

The team helps adults who are living in the Neath Port Talbot area with the support they need to retain their independence in their own homes.

This support can include acute clinical care, home alterations and equipment, physiotherapy and speech and language therapy, help with diet, help with washing and dressing and help with medicines.

How to get help

The team can support people to come out of hospital and back to their own homes. They also help to prevent hospital and care home admissions by assessing the level of need and providing the right assistance.

GPs, hospitals, district nurses and social workers are among those who can refer people to the CRT.

Patients, their families or their friends can also contact the team directly through the Neath Port Talbot Gateway. This is open Monday to Thursday between 8.30am and 5pm and Friday from 8.30am to 4.30pm.

Continuing Health Care

Patients who require increased nursing care can be assessed using the Neath Port Talbot Continuing Health Care Assessment Criteria. This is a multi-disciplinary assessment which District Nurses can facilitate. Decisions for the funding are made by the Local Health Board.

Equipment

District nurses are able to assess patients for equipment from a limited directory. Equipment is accessed via the Joint Equipment store in Neath and Port Talbot. The types of equipment available include:

  • Commodes
  • Walking frames
  • Hospital beds and pressure relieving mattresses
  • Suction devices
  • Syringe drivers

Health Promotion

District nurse are trained in health promotion and are able to assess and facilitate the health needs of the population. District nurses generally undertake health promotion on an individual basis usually within the patient’s home setting.

Nurse Prescribing

District nurses who have successfully undertaken an accredited nurse-prescribing module are able to prescribe from the Nurse Prescribing Formulary/BNF.
